The present invention provides a liposomal composition, method of using a liposomal composition, and devices and modes of operation of the devices and of the compositions, and kits related thereto. The invention provides for the reverse transport of cholesterol from peripheral tissues to the liver in a warm blooded mammal while controlling plasma atherogenic lipoprotein concentrations, including LDL concentrations. The method and mode of operation of the devices includes the step of administering an effective amount of a multiplicity of acceptors comprised of phospholipids substantially free of sterol. The method optionally includes the step of periodically assaying atherogenic lipoprotein concentrations with an assay during the treatment period to assess atherogenic lipoprotein concentrations and obtain an atherogenic lipoprotein profile, and adjusting the administration in response to said profile. The large liposomes are dimensioned larger than fenestrations of an endothelial layer lining hepatic sinusoids in the liver so that the liposomes are too large to readily penetrate the fenestrations of one variant. The therapeutically effective amounts are in the range of about 10 mg to about 1600 mg phospholipid per kg body weight per dose. A pharmaceutical composition and related kit for mobilizing peripheral cholesterol and sphingomyelin that enters the liver of a subject consisting essentially of liposomes of a size and shape larger than fenestrations of an endothelial layer lining hepatic sinusoids in the liver is also provided. The invention also provides for control of cholesterol related genes and other compounds.
L'invention porte sur le transport inverse du cholestérol des tissus périphériques vers le foie chez un mammifère avec, concurremment, régulation des concentrations de lipoprotéines athérogènes du plasma, notamment des concentrations de lipoprotéines à faible densité. La méthode et le mode de fonctionnement des dispositifs consistent à administrer une quantité efficace de plusieurs accepteurs constitués de plusieurs phospholipides essentiellement exempts de stérol. On peut, facultativement, doser périodiquement des concentrations de lipoprotéines athérogènes en procédant à un dosage au cours de la période de traitement pour évaluer les concentrations de lipoprotéines athérogènes et obtenir un profil de ces lipoprotéines avant d'ajuster l'administration en fonction de ce profil. Les liposomes de grande dimension sont plus grands que les ouvertures d'une couche endothéliale bordant les sinusoïdes hépatiques du foie, de telle sorte que ces liposomes sont, selon l'un des modes envisageables, trop grands pour pénétrer à travers ces ouvertures. Les quantités thérapeutiquement efficaces se situent dans la plage comprise entre environ 10 et 1600 mg de phospholipides par kg de poids corporel par dose. L'invention porte sur une composition pharmaceutique et sur un kit destiné à mobiliser le cholestérol périphérique et la sphingomyéline entrant dans le foie du sujet, cette composition et ce kit étant essentiellement constitués de liposomes d'un calibre supérieur aux ouvertures de la couche endothéliale revêtant les sinusoïdes hépatiques du foie. L'invention porte également sur la régulation des gènes se rapportant au cholestérol et d'autres composés.
