The invention relates to the use of a 5-HT4 receptor antagonist in the manufacture of a medicament for the prophylaxis or treatment of atrial remodelling in a mammal. Preferably, the antagonist is N-[(1.nbutyl-4- piperidinyl) methyl]-3,4-dihydro-2H-[1,3]oxazino[3,2-a]indole-10-carboxamide (SB207266) or a pharmaceutically acceptable salt thereof. The invention also relates to the use of SB 207266 or a pharmaceutically acceptable salt thereof in the manufacture of a medicament for the treatment or prophylaxis of atrial fibrillation in a mammal by administering to the mammal a daily oral or parenteral dosage regimen of about 0.2 mg to 1.0 mg of the SB 207266 or salt thereof per kg of total body weight (measured as the free base). The invention also relates to the use of SB 207266 or a pharmaceutically acceptable salt thereof in the prophylaxis or treatment of atrial arrhythmia in a mammal by administration of the SB 207266 or salt thereof on the first day at a loading dose of about 1.2 to about 2.0 times the daily maintainance dose, followed by administration of the SB 207266 or salt at the daily maintainance dose on subsequent days.
L'invention concerne l'utilisation d'un antagoniste de récepteurs 5-HT¿4? dans la fabrication d'un médicament pour prévenir ou traiter un remodelage auriculaire chez un mammifère. De préférence, l'antagoniste est du N-[(1?-n¿butyl-4-pipéridinyl) méthyl]-3,4-dihydro-2H-[1,3]oxazino[3,2-a]indole-10-carboxamide (SB 207266) ou un sel pharmaceutiquement acceptable de celui-ci. L'invention concerne aussi l'utilisation de SB 207266 ou d'un sel pharmaceutiquement acceptable de celui-ci dans la fabrication d'un médicament destiné à traiter ou à prévenir la fibrillation auriculaire chez un mammifère, par l'administration au mammifère d'une dose journalière, par voie orale ou parentérale, d'environ 0,2 mg à 1,0 mg de SB 207266 ou d'un sel de ce composé par kg de poids corporel total (mesuré comme base libre). L'invention concerne aussi l'utilisation de SB 207266 ou d'un sel pharmaceutiquement acceptable de ce composé dans la prévention ou le traitement d'arythmie auriculaire chez un mammifère, par l'administration de SB 207266 ou d'un sel pharmaceutiquement acceptable de ce composé, le premier jour à une dose initiale équivalant à environ 1,2 à environ 2,0 fois la dose d'entretien journalière ; suivie par l'administration, les jours suivants, de SB 207266 ou d'un sel pharmaceutiquement acceptable de ce composé à la dose d'entretien journalière.
