Methods and apparatus are provided for bilateral renal neuromodulation, e.g., via a pulsed electric field, via a stimulation electric field, via localized drug delivery, via high frequency ultrasound, via thermal techniques, etc. Such neuromodulation may effectuate irreversible electroporation or electrofusion, necrosis and/or inducement of apoptosis, alteration of gene expression, action potential attenuation or blockade, changes in cytokine up-regulation and other conditions in target neural fibers. In some embodiments, neuromodulation is applied to neural fibers that contribute to renal function. In some embodiments, such neuromodulation is performed in a bilateral fashion. Bilateral renal neuromodulation may provide enhanced therapeutic effect in some patients as compared to renal neuromodulation performed unilaterally, i.e., as compared to renal neuromodulation performed on neural tissue innervating a single kidney.
L'invention concerne des méthodes et un appareil de neuromodulation rénale bilatérale, par ex., au moyen d'un champ électrique pulsé, d'un champ électrique de stimulation, d'une administration de médicament locale, d'ultrasons à haute fréquence, de techniques thermiques, etc. Une telle neuromodulation peut provoquer de manière irréversible une électroporation ou une électrofusion, une nécrose et/ou une induction d'apoptose, une altération d'expression génique, une éventuelle atténuation d'une action ou un éventuel blocage d'une action, des changements dans la régulation à la hausse de cytokines ou d'autres conditions dans des fibres neurales cibles. Dans certains modes de réalisation, la neuromodulation est appliquée à des fibres neurales qui contribuent à la fonction rénale. Dans d'autres modes de réalisation, la neuromodulation est réalisée de manière bilatérale. Une neuromodulation rénale bilatérale peut avoir un effet thérapeutique accru chez certains patients comparativement à une neuromodulation rénale unilatérale, à savoir, comparativement à une neuromodulation rénale effectuée sur du tissu neural innervant un seul rein.
