This is an instrument (10) for applying tension to a metal surgical cable (4) used in certain surgical procedures. The cable tensioner instrument (10) has a ratchet feature (40, 88) which has the ability to steadily increase the tension in the cable. Also included is a scale (91) incorporated into the cable tensioner (10) so that the tension in the cable is directly measured and indicated at all times. Finally, the cable tensioner (10) is ergonomically designed into an elongated cylindrical shape to easily fit in the cupped hand of a surgeon.
