A bend die assembly (10) includes a bend die (16) and a clamp block assembly. The bend die is reversible and includes a T-shaped projection (130) which extends outward from the bend die. The clamp block assembly includes a first clamp block (18) and a mating second clamp block (20). The first clamp block includes a first T-shaped opening (150) sized to slidably receive the bend die T-shaped projection. Each clamp block includes a tube groove (120, 202) which includes a plurality of indentations (194) corresponding to a desired compound bend to be formed in tubing (12).
