A surgical access apparatus includes a liner base and an access housing for positioning outside the body. The liner base includes an inner member adapted for insertion through an opening within body tissue for positioning within the body; a sleeve member connected to the inner member and dimensioned to extend from the inner member through the opening within the body tissue, and a plurality of tensioning elements connected to the inner member and associated with the sleeve member to impart a tensioning effect on the sleeve member. The access housing includes a first element and a second element. The second element is operatively connected to the tensioning elements and adapted for rotational movement relative to the first member to cause the tensioning elements to displace the inner member toward the access housing and to cause the sleeve member to engage and retract tissue defining the opening within the body. The tensioning members are adapted to move relative to the sleeve member upon rotation of the second element and may be embedded within the sleeve member. A seal is preferably mounted to the access housing and is adapted to form a fluid tight seal about an object inserted therethrough. A zero closure valve may also be mounted relative to the liner member for forming a fluid tight seal in the absence of an object positioned therethrough. The second member of the access housing is adapted to be selectively secured at a predetermined rotational relationship with respect to the first member to provide for selective tensioning of the tensioning elements and selective retraction of tissue.
L'invention concerne un appareil d'accès chirurgical comprenant une base de revêtement et un logement d'accès destiné à être positionné à l'extérieur du corps. La base de revêtement comprend un élément intérieur conçu pour permettre une insertion à travers une ouverture à l'intérieur du tissu corporel permettant un positionnement à l'intérieur du corps; un élément de manchon relié à l'élément intérieur et dimensionné pour s'étendre depuis l'élément intérieur à travers l'ouverture jusqu'à l'intérieur du tissu corporel, ainsi qu'une pluralité d'éléments de tension reliés à l'élément intérieur et associés à l'élément de manchon afin de conférer un effet de tension sur l'élément de manchon. Le logement d'accès comprend un premier élément et un second élément. Le second élément est relié fonctionnellement aux éléments de tension et conçu pour avoir un mouvement rotatif par rapport au premier élément de manière à amener les éléments de tension à déplacer l'élément intérieur vers le logement d'accès et à amener l'élément de manchon à venir au contact du tissu et à rétracter le tissu définissant l'ouverture à l'intérieur du corps. Les éléments de tension sont conçus pour se déplacer par rapport à l'élément de manchon lors de la rotation du second élément, et ils sont intégrés dans l'élément de manchon. Un joint est monté de préférence sur le logement d'accès et il est conçu pour former un joint étanche au fluide autour d'un objet inséré par celui-ci. Une soupape de fermeture à l'état zéro peut aussi être montée par rapport à l'élément de revêtement pour former un joint étanche au fluide en l'absence d'un objet positionné dans celui-ci. Le second élément du logement d'accès est conçu pour être fixé sélectivement en une relation rotative prédéterminée par rapport au premier élément, afin d'exercer une tension sélective des éléments de tension et une rétraction sélective des tissus.
