Methods and devices providing transmyocardial blood flow or coronary revascularization for the treatment of coronary atherosclerosis and resulting myocardial ischemia by increasing the flow of blood from one or more oxygenated blood sources within the patient to one or more sites selected in the arterial vascular system of the heart using a channel (42) for maintaining and regulating blood flow therebetween. A valved conduit (48) or a self- maintained channel (42) is created between the left ventricle (20) reservoir of oxygenated blood and the coronary artery (26) distal to an area of obstruction by surgical and percutaneous methods. Preferably, the conduit (48) or self-maintaining channel (42) integrally regulates the flow of blood between the oxygenated blood source and the site selected in the arterial vascular system of the heart wherein an increase in blood flow is desired.

L'invention concerne des procédés et dispositifs assurant un flux sanguin transmyocardiaque ou une revascularisation coronaire pour le traitement de l'athérosclérose coronaire et de l'ischémie transmyocardiaque. Ces procédés et ces dispositifs permettent d'augmenter, chez un patient, le flux sanguin provenant d'au moins une source de sang oxygéné, sur au moins un site sélectionné dans le système vasculaire artériel cardiaque. Cette intervention est pratiquée au moyen d'un canal (42) permettant de maintenir et de réguler le flux sanguin entre cette source et ce site. Un conduit (48) muni d'une soupape ou un canal autonome (42) est introduit, par voie chirurgicale et percutanée, entre le réservoir de sang oxygéné du ventricule gauche (20) et l'artère coronaire (26), qui sont distaux par rapport à une zone d'obstruction. De préférence, le conduit (48) ou canal autonome (42) régule intégralement le flux sanguin entre la source de sang oxygéné et le site sélectionné dans le système vasculaire artériel cardiaque où une augmentation du flux sanguin est souhaitée.
