The invention provides a rotational atherectomy system, device and method comprising a flexible, elongated, rotatable drive shaft with an abrasive section within a pre-curved section of the drive shaft. The device may further comprise a concentric or eccentric enlarged diameter section that is at least partially covered with abrasive material to comprise the abrasive section. The abrasive section may further comprise an abrasive crown or burr mounted to the drive shaft. The pre-curved drive shaft allows smaller diameter and/or massive abrasive regions to be used while sweeping larger diameters during high-speed rotation. The pre-curved region is substantially straightened for insertion into vasculature and placement adjacent stenosis by insertion of the guide wire. Removal of guide wire proximally from the pre-curved region allows the drive shaft to return to its pre-curved form for ablation. Reinsertion of the guide wire beyond the pre-curved region straightens the drive shaft for ease of removal.
L'invention concerne un système, un dispositif d'athérectomie rotatifs et un procédé apparenté, le système comprenant un arbre d'entraînement flexible, allongé, rotatif, une section abrasive se trouvant à l'intérieur d'une section précourbée de l'arbre d'entraînement. Le dispositif peut également comprendre une section à diamètre élargi, concentrique ou excentrique, qui est au moins partiellement recouverte par un matériau abrasif pour comprendre la section abrasive. La section abrasive peut en outre comprendre une fraise ou une couronne abrasive montée sur l'arbre d'entraînement. L'arbre d'entraînement précourbé permet à des régions abrasives de diamètre plus petit et/ou massives d'être utilisées tout en balayant des diamètres plus grands pendant la rotation à haute vitesse. La région précourbée est sensiblement redressée pour être introduite dans un système vasculaire et placée à côté d'une sténose par introduction du fil de guidage. Le retrait du fil de guidage à proximité de la région précourbée permet à l'arbre d'entraînement de retourner à sa forme précourbée pour une ablation. Une réintroduction du fil de guidage au-delà de la région précourbée redresse l'arbre d'entraînement pour un retrait facilité.
