An implantable cardiac stimulator (10), which may include cardioversion (62) and pacemaker (30, 32) capabilities, which has apparatus (74) for detecting an externally applied shock, such as a defibrillation or cardioverting shock. In response to detection of such a shock, the cardiac stimulator has apparatus to adjust the parameters of applied therapy, or to select alternative therapy, including, but not limited to, adjusting the magnitude of stimulus pulses for a predetermined length of time. The risk of loss of capture, and consequent failure of cardiac function, following an externally applied shock is thereby reduced.

L'invention concerne un stimulateur cardiaque implantable (10) qui peut présenter des fonctions de cardioversion (62) et de stimulation cardiaque (30, 32) et qui comporte un appareil (74) pour la détection d'un choc appliqué depuis l'extérieur, tel qu'un choc de défibrillation ou de cardioversion. En réponse à la détection d'un choc de ce type, le stimulateur est doté d'un appareil qui ajuste les paramètres de la thérapie appliquée, ou qui sélectionne une autre thérapie, dont, entre autres, l'ajustement de l'ampleur des impulsions stimuli pendant une durée prédéterminée. Le risque d'affaiblissement de la capture et de défaillance résultante de la fonction cardiaque à la suite d'un choc appliqué depuis l'extérieur est ainsi réduit.
