A sleeve secures a first axial portion of the bolt to a first portion of the circumferential wall. Grout secures a second axial portion of the bolt means to a second portion of the circumferential wall; A substantially disk shaped plug limits a passage of grout to between the first axial portion of the bolt and the first portion of the circumferential wall.
