Samples are tested for mutations in the BRCA1 gene using a hierarchical approach. First, each sample is amplified in one or more multiplex PCR amplification reactions. Each multiplex PCR reaction produces a mixture of amplified fragments. The sizes and amounts of these fragments are evaluated and compared to standard values reflecting the sizes and amounts of fragments produced when the same multiplex amplification is performed on the wild-type BRCA1 gene. Differences between the observed fragment sizes and/or amounts and those for the wild-type gene are indicative of a mutation with the BRCA1 gene of the sample. Next, one or more of the exons of the BRCA1 gene are sequenced, preferably only for those samples where no mutation was detected by analysis of the multiplex PCR fragments. The sequencing procedure can be performed by amplification and sequencing of the multiplex amplification mixture.

On procède sur des échantillons à des tests de détection de mutations sur des gènes BRCA1 par une approche hiérarchique. Tout d'abord chaque échantillon est amplifié au cours d'une ou de plusieurs réactions d'amplification PCR multiplexée. Chacune des réactions d'amplification PCR multiplexée produit un mélange de fragments amplifiés. La taille et la quantité de ces fragments est évaluée et comparées à des valeurs de référence reflétant la taille et la quantité des fragments obtenus lorsque la même amplification multiplexée est effectuée sur un gène BRCA1 de type sauvage. Les différences entre la taille et/ou la quantité des fragments observés et celles des gènes de type sauvage indique une mutation du gène BRCA1 de l'échantillon. On procède ensuite à un séquençage d'un ou plusieurs des exons du gène BRCA1 ne portant de préférence que sur ceux des échantillons où aucune mutation n'a été décelée par analyse des fragments produits par PCR multiplex. Le processus de séquençage peut s'effectuer par amplification et séquençage du mélange obtenu par amplification multiplex.
